All services offered by Harewood Park are client led. Clients have complex needs and are not functioning by reason of mental illness and /or a mild learning disability. They lack self confidence, have poor self esteem and are unable to make decisions or take responsibility for self.

Prior to admission Harewood Park will receive background information, existing care plans and risk assessments from the referring agencies.

Clients visit Harewood Park prior to admission giving them the opportunity to see the environment for themselves and meet staff and residents which allows them to make an informed choice to come.

Initial Assessment

Our process commences with an initial assessment of their needs which usually takes between 4 to 6 weeks to complete.

This is followed by a Care Programme Approach (CPA) meeting where the decision is taken, with the multidisciplinary team, referring agency and client, that Harewood Park is a suitable placement.

Rehabilitation Programme

Programmes are based on individual need and determined by the client’s ability and at a pace they are comfortable with.

Clients begin on the Pathway to Rehabilitation by working with staff on a 1:1 basis to look at the areas which are prevent them from functioning and to develop a routine for daily living, including:

  • Personal Care
  • Care of their room
  • Personal laundry
  • Cooking skills
  • Money management

Programmes are assessed daily and evaluated monthly

Areas that need development are identified jointly between the staff member and client.

Treatment

The role of the qualified nurse:

  • To observe and monitor the client’s mental health, functioning levels and medication
  • To formulate care plans and risk assessments
  • To identify client’s needs including behavioural problems, which prevent progress
  • Cognitive/diversional tecniques
  • Counselling on a 1:1 basis
  • Anger and anxiety management
  • Relaxation

Clients are involved in all aspects of their care and are encouraged to sign their care plans and risk assessments to acknowledge their input.

Additional services can be requested and facilitated:

  • Psychological and Behavioural Services.

Promotion of Personal Growth and Development

  • Staff encourage participation in social and leisure activities aiming to improve the self esteem and confidence of our clients.
  • We aim to give our clients a focus externally to themselves by engaging in-house activities such as the house rota’s, table games, arts and crafts, promoting interaction with other clients within the house.
  • We encourage our clients to experience the local community and increase their confidence by use of the leisure centre, library, public house, café’s and local shops.  Over time our clients will use these amenities independently or in the company of other clients.
  • These ultimately lead to participation in voluntary work, attendance at college and access to locally based facilities such as “Rethink”.

Reviews / Assessments

Throughout the client’s stay they will be seen regularly by their consultant psychiatrist.  Also reviews are held with the referring agency and multi disciplinary team to monitor progress, identify any areas which need additional rehabilitation and to plan for the ultimate discharge of the client.  This will include identifying the level of care that they will need and type of suitable accommodation in the community.
